Find us on 
FROM THE NORTH: At junction of U.S. 6 and Indiana State Road 15 at the north edge of Kosciusko County, travel south approximately one and one-half miles to Milford. Our complex is on both the east and west side of 15. Main reception is in the west building.
FROM THE SOUTH: At junction of U.S. 30 and Indiana State Road 15 in Warsaw, travel north approximately ten miles through Leesburg to Milford. Our complex is just north of the second stoplight in Milford on both the east and west side of 15. Main reception is in the west building.
